Not every viral food trend is a nutrition story. Some are trauma-surgery stories.
What follows is an aggregation of trends that have put people — mostly children — in hospital, with the mechanism in each case and a source. This is assembled from published case reports, hospital statements and news reporting rather than original reporting of our own.
One thing became obvious while compiling it: the injuries cluster around heat, blades, and concentration — trends where a normally-safe ingredient is made dangerous by a technique, not by its nutritional content.
Molten sugar — "glass fruit" tanghulu
The injury: deep second- and third-degree scald burns, typically to hands and face, some requiring skin grafts.
Shriners Children's Boston raised the alarm in August 2024, and the trend has not gone away: in September 2025 the hospital reported treating eight patients that year, and described "dozens of children" hospitalized with second- and third-degree burns during 2024.
Why it is worse than boiling water. Three things compound. Molten candy runs around 350°F, against water's 212°F ceiling. Sugar has a high heat capacity, so it holds that energy. And it is viscous — it sticks rather than running off, extending contact time. The result, as the hospital describes it, is "one or several small but very deep burns, often in critical areas such as the hand or the face."
The microwave is the specific aggravating factor: containers melt, superheated glass shatters, and children lift bowls of molten sugar.
First aid: cool running water for at least 20 minutes. Not ice.
Mandoline blades — the viral cucumber salad
The injury: deep hand lacerations and fingertip injuries; ER visits for stitches.
Mechanism: a mandoline's blade is driven by the user's own force. As the cucumber shortens, the hand tracks toward the blade plane and shears through in a single pass — faster than a pain reflex can withdraw it. The hand guard is the only barrier, and it is the first thing people skip.
We are giving no injury statistics here, because we could not verify any. The confident-looking figures circulating for mandoline ER visits trace back to SEO content pages, not to any injury surveillance data we could find.
Dry chia seeds — "internal shower" drinks
The injury: complete oesophageal obstruction requiring emergency endoscopy.
Mechanism: chia seeds form a cohesive gel when hydrated. Swallowed dry and chased with water, they expand inside the oesophagus rather than in the glass. In a documented case, a 39-year-old man swallowed about a tablespoon dry, followed by water; the gel wedged at a narrowing and defeated standard extraction — neither solid enough to grasp nor liquid enough to suction. The team worked for several hours, eventually using a neonatal endoscope.
The gastroenterologist's advice is the whole lesson: "Nobody should be eating these seeds dry. Let them expand fully in some kind of liquid first."
An honest caveat: that case dates from 2014, and the "internal shower" trend emerged around 2022. The case demonstrates the mechanism the trend can trigger. It was not caused by it, and we are not going to imply otherwise.
Borax
The injury: nausea, vomiting, diarrhoea, abdominal pain and rash; with heavier exposure, anaemia, seizures and kidney damage.
Mechanism: borax hydrolyses in stomach acid to boric acid, which irritates the gut directly and is cleared almost entirely by the kidneys, where it concentrates. It is classified in the EU as a reproductive toxin.

Capsaicin — the One Chip Challenge
The injury: in September 2023, the death of Harris Wolobah, aged 14.
Mechanism: extreme capsaicin concentrations cause severe pain and forceful repeated vomiting alongside a sympathetic surge — chest pain, palpitations, hypertension, coronary vasospasm. The medical examiner recorded cardiopulmonary arrest following ingestion of a food substance with high capsaicin concentration. The autopsy also found an enlarged heart and a congenital heart defect.
That last detail is important and is usually dropped. The chip did not need to be lethal to a typical teenager to have been lethal to this one — which is exactly the problem with a challenge administered at scale to strangers.
Dry scooping pre-workout powder
The injury: choking and powder aspiration; in one published case, a heart attack.
Mechanism: swallowing concentrated stimulant powder undiluted delivers the full dose as a rapid bolus. A case report describes a previously healthy 25-year-old man with thrombotic occlusion of the proximal left anterior descending artery — and angiography showing no atherosclerosis and no dissection. A clean artery, closed by a clot.
What the pattern says
None of these trends is about what food does inside the body over years, which is what nutrition argument usually concerns. They are about a technique applied to an ordinary ingredient: sugar heated past its hard-crack stage, a blade without its guard, a hydrocolloid hydrating in the wrong place, a stimulant taken undiluted.
That may be why they spread so well. The visual payoff is immediate — the shattering sugar shell, the rhythmic slicing, the crunch — and immediacy is what recommendation algorithms reward. The injury, when it comes, arrives in the same instant, and is not filmed.
